We provide customized services to young adults transitioning back into the community, transitioning to an independent living situation (emancipation) or a college living environment. Our goal is to provide a seamless transition and comprehensive wrap around services in order to empower the student to be self-sufficient as they reach this important milestone, teaching them the life skills they need to be successful.

As adolescents move into young adulthood, it is natural that they look for opportunities to live independently of their families.  Leaving the comfort of home is usually the last step taken before officially joining the adult world.  This transition may be more complicated for young adults who lack the experiences, skills or confidence to take that step.

Recognizing this challenge, PRN for Families is introducing Independent/College Transition services to help young adults make the transition from relying on their parents and family to an independent living situation.

These services include Intensive in-home crisis intervention; Collaboration with schools, programs and consultants; an Initial Assessment; comprehensive planning and case management; counseling and consultation; Life-skills training w/followup and on going support.
